What Form I-693 is
If you are applying to adjust your status to lawful permanent resident, which most people call getting a green card, USCIS uses Form I-693 to establish that you are not inadmissible to the United States on health-related grounds. The full name is Report of Immigration Medical Examination and Vaccination Record.
The USCIS page for the form, last reviewed on July 24, 2026, says that people filing Form I-485 must also submit Form I-693. If you are required to submit the form, or a partial form such as the Vaccination Record, you must submit it with your Form I-485.
Who fills it in
You do not complete the medical part yourself. A civil surgeon does. A civil surgeon is a doctor designated by USCIS to perform immigration medical examinations. USCIS has a Find a Civil Surgeon tool on its website for locating one near you.
- The civil surgeon gives you the completed Form I-693, signed and placed in a sealed envelope.
- USCIS says: do not accept the Form I-693 if it is not in a sealed envelope.
- The fee for the examination is set by the civil surgeon and varies from one doctor to another.
Vaccines USCIS lists
The USCIS vaccination requirements page lists the vaccines required for adjustment of status applicants:
- Mumps, measles, rubella.
- Polio.
- Tetanus and diphtheria toxoids.
- Pertussis.
- Haemophilus influenzae type B.

You are required to document the vaccines that are age appropriate for you. The civil surgeon is required to administer the flu vaccine from September 1 until March 31. On COVID-19, USCIS states that as of January 20, 2025, applicants for adjustment of status are no longer required to provide documentation of having received the COVID-19 vaccination.
Your vaccination records from home
USCIS explains that the civil surgeon reviews your vaccination records at the examination to see whether you have proof of the required vaccinations. It asks you to take any written vaccination documentation you may have to the civil surgeon. If you have written evidence of immunity, you should take that too.
For people from Ukraine, Russia, and other post-Soviet countries, that documentation is usually one of these:
- The preventive vaccination card, form 063/o in Ukraine and form 063/u in Russia.
- The vaccination certificate, a small booklet, form 156/u in Russia.
- The child's medical card or an extract from it.
- An international certificate of vaccination, if you have one.
These records are handwritten in Cyrillic, often with abbreviations and brand names of vaccines. An English translation lets the civil surgeon read the dates and the vaccine names and see which vaccinations are already documented.
Other documents for the exam
The civil surgeon's office tells you what to bring. Offices commonly ask for a government photo ID and any medical records about chronic conditions or past treatment. If those records are in Russian or Ukrainian, translate the parts the doctor needs: diagnoses, treatment, dates, and test results.
A waiver of the vaccination requirement
USCIS states that a waiver may be available only when you are opposed to all vaccinations in any form, your objection is based on religious beliefs or moral convictions, and those beliefs are sincere. A waiver may not be granted if you object only to specific vaccinations. Whether to ask for a waiver is a question for an immigration attorney.
