Doctor's terms, kept exactly

Medical records are dense with abbreviations, handwriting and drug names. The translation renders each term as written, keeps units and codes unchanged, and marks anything unreadable instead of guessing.

Diagnoses and codes

Diagnoses are translated as written, with ICD codes kept as printed so a US doctor can read them directly.

Units not converted

Lab values stay in the original units, with the unit named. Conversion is a clinical decision, not a translation one.

Drug names

Medications are rendered by their international nonproprietary name where the record gives one, with the brand name kept as printed.

Clinic stamps and signatures

The clinic's name, the doctor's stamp and the signature block are translated or described, page by page.

Which version do you have

Discharge summary or clinic certificate, both translated in full

Most medical records sent to us are one of two things: a discharge summary written after a hospital stay, or a certificate or extract issued by a clinic on a numbered form. The approach is the same; the length is not.

Hospital discharge summary

Several dense pages
  • Two to five typed pages with the hospital's header, a diagnosis block, treatment and recommendations
  • Admission and discharge dates, diagnoses with codes, procedures, medications, lab values and signatures

Medical record translation, the questions we hear most

Will the civil surgeon accept this for the immigration exam?

The civil surgeon reads the translated record to see what was done and when. The certified translation gives them that; what they accept as proof is their clinical decision.

The record is handwritten and hard to read. What then?

Whatever is legible is translated; unreadable fragments are marked as such. If a key item is unclear, we ask you for a better photo or the clinic's typed copy first.

Do you translate lab reports and imaging descriptions?

Yes, each as its own page, with values and units as printed.

Will USCIS accept this translation?

It follows what USCIS asks for: the whole document in English plus a signed statement that the translator is competent and the translation is complete and accurate. Every order includes that statement, signed and dated by the translator.
