What matters in this document

Two names, one person

The certificate lists the name before and the name after, the date of the change and the registry that recorded it. The translation has one job: to make the link between your documents obvious to a reader who has never seen either name.

Old name, new name

The previous name follows the spelling in your older documents; the new one follows your passport. Both are stated once in a translator's note.

Date of change

The date the change was registered is converted to the US format and kept distinct from the certificate's issue date.

The office, the act record number and the seal are translated or described.

The link between an old name and a new one is usually a certificate from the civil registry. Where the change went through a court, or the registry corrected a record by court order, the court decision is the document instead.

I changed my surname by marriage. Do I need this document?

Usually not; the marriage certificate records the surname taken, and its translation is the link. This page is for changes registered separately.

My name was changed in childhood and only a stamp on the birth certificate shows it. What do you translate?

The stamp, as part of the birth certificate translation. If you also obtained a certificate or extract about the change, send it and it is translated as its own page.

Which spelling do you use for the old name?

The one in the documents that carry it, such as an old passport or diploma. Tell us when you order; if there is none, standard transliteration is used and noted.

Will USCIS accept this translation?
