Order NowInside a Russian medical record: the details that decide it
Russian clinical writing leans on abbreviations, Latin drug names in Cyrillic and a stamp for certificates that reads for references. The translation keeps codes and units, expands abbreviations where the meaning is certain, and flags the rest.
Form numbers and fields
Forms 027/u, 086/u, extracts from the 003/u chart and others are translated with their form number and each field label.
Units and codes kept
ICD-10 codes, lab units and reference ranges are copied as printed.
Triangular and round stamps
The clinic's stamps, the doctor's personal stamp and the signature block are translated or described.
Patient's name with patronymic
Your name keeps its patronymic and matches your passport spelling.
Discharge summary or certificate 086/u, which form is yours
Russian records follow numbered forms: the discharge summary or extract on form 027/u after treatment, and the certificate 086/u or a short clinic certificate for study, work or travel. Both are translated field by field.
Discharge summary or form 027/u extract
After a hospital stay- Typed pages with the hospital's header, a triangular stamp and the signatures of the doctor and department head
- Admission and discharge dates, diagnoses with ICD-10 codes, treatment, lab values and recommendations
- Every section is translated in full; units, codes and drug names are kept as printed
- Asked for by US doctors, civil surgeons, courts and insurers
Send every page, including the signature page and the stamp.
Certificate 086/u or clinic certificate
For study or work- A one-page form with fixed fields, partly handwritten, with the clinic's stamp and the doctor's personal stamp
- Specialists' findings, a fitness statement, vaccination notes and the issue date
- Field labels, entries and both stamps are translated; unreadable handwriting is marked
- Asked for by universities, employers and immigration medical exams
Send a straight-on photo of the whole form, both sides.
The whole form, both sides and the margins
Back page and margins
The back of the form, where visits, stamps and notes often continue, is translated as part of the same page, and margin notes are included.
Handwriting, marked when unreadable
Handwritten entries are translated as written; any word that cannot be read is marked as illegible rather than guessed.
Every date converted
Admission, discharge and visit dates are converted to month-day-year and kept beside the event they belong to.
Doctor, department, titles
The treating doctor, the department head and the department name are translated, with medical titles rendered literally.
One translation, accepted where you need to file
The same certified file works for immigration, courts, universities and employers. Say who it is for when you order and the certificate is worded for that office.
USCIS
Green card, citizenship and asylum filings. Certification statement on every page set.
Immigration courts
Same certified translation, laid out page by page so the clerk can compare it with the original.
Universities and WES / ECE
Admissions offices and credential evaluators get the certified English file with the original attached.
Employers and licensing boards
HR departments get the translation with the certificate, ready to attach to an application.

Price My DocumentRussian medical record, the questions we hear most
Can you translate the doctor's handwriting?
Usually, since the forms give context. Anything unreadable is marked, and if it is important we ask for a typed extract from the clinic.
Do you convert lab units?
No. Units stay as printed and are named; a US doctor converts if needed.
Is form 086/u the medical certificate for university?
Yes. It is the certificate of health for study or work, and it is translated field by field with the stamps.
Will USCIS accept this translation?
It follows what USCIS asks for: the whole document in English plus a signed statement that the translator is competent and the translation is complete and accurate. Every order includes that statement, signed and dated by the translator.
How much does it cost and how fast is it?
From $23.70 per document, certification included. A one-page document is usually ready the next business day; the date is confirmed before you pay.
