Cards issued by the service centers of the Ministry of Internal Affairs list categories with the date each was opened, an expiry date on newer cards and a field for restrictions. Older booklets and cards without an expiry are still valid and still translated.
Latin spelling as on the card
The card's Latin name follows the Ukrainian passport standard and usually matches your passport. If it does not, the passport spelling wins and the card's version is noted.
Category dates and expiry
Each category carries its own opening date; newer cards add an expiry. All dates are converted to the US format.
Issuing service center
The name and code of the service center are translated and kept as printed for verification.
Old forms too
Licenses issued before 2005, in the paper booklet or on the first laminated cards, follow different layouts; the translation mirrors the one you send.
Plastic card or Diia, the card is what we translate from
Ukrainian drivers hold a plastic card from a service center and, more recently, a digital copy in Diia. The digital copy is convenient, but the certified translation is made from the card itself.
Plastic card from a service center
Most common today- A credit-card-size card with a photo, field labels in Ukrainian and English, a Latin transliteration of the name and a category table on the back
- Name with patronymic, date and place of birth, dates of issue and expiry, the service center code, the license number, categories with their opening dates and a restrictions field
- Both sides, field by field; the printed English labels are checked, the Ukrainian values translated, categories kept as letters
- Asked for by the DMV, insurers, rental companies and employers; by USCIS as a supporting identity document
Send the front and the back as two straight photos; the category dates on the back must be readable.
Digital license in Diia
Phone app version- A screen in the Diia app with your photo, the license fields and a QR code, or a PDF copy shared from the app with a verification block
- The same fields as the card, drawn from the register, plus the date the copy was generated and a verification code
- The PDF copy in full, including the verification block; a screenshot is described as a screenshot, not as a document
- Useful when the card is lost or abroad; offices in the US generally want the physical card, so check before you rely on it

Printed English is not a translation, here is what we add
Values behind the English labels
The card's labels are bilingual, but the values, the issuing service center and any notes are in Ukrainian. Each is translated so the card reads fully in English.
Older licenses with stamps
Licenses issued before the card carry stamps against opened categories and a stamp of the issuing office. Each is translated in place.
Place of birth in two scripts
The card prints the place of birth in Ukrainian and in Latin letters by its own standard. Both are shown, so the license matches the birth certificate and passport.
Restrictions and remarks
The restrictions field and any remark added by the service center are translated and explained, not copied as codes.

Will the DMV accept this translation?
The translation is certified and formatted for official use. Whether a foreign license can be exchanged or credited is decided by the DMV of your state; check its rules before you apply.
My license has no expiry date. Is that a problem?
Licenses issued before the expiry rule are valid in Ukraine without one, and the translation simply states that the field is empty. What the DMV does with it is a DMV question.
Do you translate the back with the categories?
Yes, both sides on one page, at the price of one page.
Will USCIS accept this translation?
It follows what USCIS asks for: the whole document in English plus a signed statement that the translator is competent and the translation is complete and accurate. Every order includes that statement, signed and dated by the translator.
