Ukrainian medical record translation, certified and exact
Ukrainian records come as numbered paper forms, such as the discharge summary 027/o or the certificate 086/o, and increasingly as printouts from the eHealth system through Helsi or similar apps. Both are translated in full with a signed certificate of accuracy.

Order NowUkrainian medical record, line by line: what the reviewer compares
Paper forms carry the clinic's stamp and the doctor's signature; eHealth printouts carry a record number and a QR code instead. Older records may be written in Russian, and the translation says which language each page was in.
Forms and eHealth extracts
Forms 027/o, 086/o and extracts from the electronic health record are translated with their form or record number.
Units and codes kept
ICD-10 codes, lab units and reference ranges are copied as printed.
Stamps or QR codes
Clinic stamps, the doctor's stamp and the QR or e-signature note are translated or described.
Ukrainian spelling first
Your name is transliterated to match your passport, with older Russian-language pages noted.
Paper form 027/o or eHealth printout, which one do you have
Ukrainian records come as paper forms, such as the discharge summary 027/o or the certificate 086/o, or as printouts from the electronic health system through Helsi or a similar app. Each is translated in full.

The whole record, paper or electronic
Back page and margins
The back of a paper form and any notes in the margins are translated as part of the same page.
Handwriting, marked when unreadable
Handwritten entries are translated as written; illegible words are marked, not guessed.
Dates converted, events kept together
Each date is converted to month-day-year and stays beside the visit or event it belongs to.
Doctor, department, facility
The treating doctor, the department and the facility name are translated, with medical titles rendered literally.
One translation, accepted where you need to file
The same certified file works for immigration, courts, universities and employers. Say who it is for when you order and the certificate is worded for that office.
USCIS

Price My DocumentUkrainian medical record, the questions we hear most
Can you translate a printout from Helsi or the eHealth system?
Yes. The record number, the doctor, the diagnosis and the QR note are translated, and the file states that the original is electronic.
Some pages of my record are in Russian. Is that fine?
Yes. Each page is translated from its own language at the same price, and the file says which is which.
Do you convert lab units?
No. Units stay as printed and are named.
Will USCIS accept this translation?
It follows what USCIS asks for: the whole document in English plus a signed statement that the translator is competent and the translation is complete and accurate. Every order includes that statement, signed and dated by the translator.
How much does it cost and how fast is it?
From $23.70 per document, certification included. A one-page document is usually ready the next business day; the date is confirmed before you pay.
Twenty document types, two languages
Each document and each language has its own page with its own specifics and price. Something else? Upload it and we quote it by hand.
