Ukraine has changed its certificate form more than once, and the registry itself was renamed from RATsS to DRATsS. Each version has its own details, and the translation follows the one in front of us.
Ukrainian spelling first
Names are transliterated from the Ukrainian text the way the Ukrainian passport does it, with the Russian side of a bilingual form checked for entries filled in only there.
Two passports, two spellings
Each spouse is spelled as in that person's own passport, even when the two passports use different transliteration rules.
DRATsS seals and apostille
Registry seals, the act record number and the apostille from the Ministry of Justice are translated on the same order.
Dates in the US format
Day-first dates are converted to month-first, and the original stays in brackets where it matters.

Ukraine has reprinted its marriage certificate more than once. The form you hold determines which text is the source and what the English file says about it.
Modern DRATsS form
Most common today- A single sheet with the Ukrainian trident, a series and number and printed labels in Ukrainian only
- Both spouses with patronymics, dates and places of birth, citizenship, the surnames after marriage, the act record number and date, and the issuing office
- The full page: every field, the form text, the seal, the signature block and any stamp on the back about a divorce or a duplicate
- Asked for in spousal petitions, adjustment of status, surname changes and by employers and insurers

Bilingual form from the nineties
Ukrainian and Russian text- A sheet or folded form with each field label printed twice, in Ukrainian and Russian, and entries filled by hand or typewriter in one of the two
- The same record: both spouses, dates of birth, surnames after marriage, the act number, the RATsS office and the date
- The Ukrainian text as the source, with the Russian side checked for entries filled in only there, and a note that the original is bilingual
- Asked for by USCIS in the same cases; couples married in the early nineties often hold this form

Form text, series and number
The printed wording of the form, the trident and the series and number are copied or described, so the translation identifies the same physical certificate.
Birth dates and citizenship of both spouses
Each spouse's date and place of birth and citizenship are translated as printed and converted to the US format, then checked against the passports you send.
Divorce stamp or duplicate note
If the back carries a stamp about a later divorce or a note that this is a duplicate, it is translated in place with its date and office.
Handwritten and typewritten entries
Entries typed on a typewriter or written by hand are transcribed as written; a correction by the registrar is marked as a correction.

Will USCIS accept this translation?
It follows what USCIS asks for: the whole document in English plus a signed statement that the translator is competent and the translation is complete and accurate. Every order includes that statement, signed and dated by the translator.
My certificate is bilingual, Ukrainian and Russian. Which text do you use?
The Ukrainian text is the source, and we check the Russian side for anything filled in only there. The English file notes that the original is bilingual.
Do I need an apostille for USCIS?
USCIS does not ask for an apostille on civil documents; it asks for a certified translation. If you already have one from the Ministry of Justice, we translate it with the certificate.
The surname on the certificate is not the one in my passport. What do you do?
The certificate is translated as written, including the line that records which surname you took. That line is what explains the change to the officer, so nothing is adjusted to match.
