Ukrainian records use abbreviations such as AKDP, BTsZh, KPK, OPV, IPV and Hib, and often appear on a bilingual Soviet form or as a Helsi printout. Mantoux results in millimeters and lot numbers are kept as written.
Abbreviations mapped
AKDP becomes DTP, BTsZh becomes BCG, KPK becomes MMR, with the Ukrainian abbreviation kept in brackets.
Dates and dose markers
Each dose date is converted, and dose markers such as V1 or RV1 are explained in a note.

My COVID-19 certificate from Diia is already in English. Do I need anything?
If it is fully in English, no. If you have a paper certificate in Ukrainian, it is translated as its own page.
Some entries are in Russian on an older card. Is that fine?
Yes. Each line is translated from its own language at the same price.
Will the civil surgeon accept this translation?
The surgeon needs a readable record of each vaccine and date, and that is what the certified translation gives. Which doses they credit is their clinical decision.
